Scope of this website
crownclinical.org is an informational website. It is not a portal for protected health information (PHI). Clinical intake, telehealth visits, document exchange, and other PHI-bearing activity take place on a separate platform operating under a Business Associate Agreement (BAA).
What we collect on this site
The contact form on this site collects only the information necessary to respond to a non-clinical inquiry: name, email, optional phone number, and a brief reason for contact. We ask that you do not include diagnoses, symptoms, medications, or other health details in this form.
HIPAA-aware handling
Where clinical communication occurs, it is conducted through platforms that maintain appropriate safeguards, including encrypted transmission and storage, access controls, audit logging, and a signed BAA with the vendor. This website itself is not a substitute for those systems.
Cookies and analytics
This site uses only the minimum cookies necessary for navigation. If analytics are added in the future, IP addresses will be anonymized and no PHI will be tracked.
Your rights
You may request that we delete the inquiry information you submitted through this site by contacting us at contact@crownclinical.org. Patients of the clinical practice have additional rights under HIPAA, which are described in full in the Notice of Privacy Practices provided at intake.
Emergencies
If you are experiencing a medical emergency, call 911. Crown Clinical does not provide emergency or urgent care and does not monitor this website in real time.
